SCHOLARSHIP APPLICATION
Sponsored by Delta Phi Chapter

Each year Delta Phi Chapter offers one or two scholarships to qualified education majors.

1. Applicant must be resident of Harrison or Tuscarawas County, Ohio.
2. Applicant must be on schedule to graduate with a degree in Education by September, 2009.
3. Applicant must provide a current transcript from their college or university.
4. The Recommendation Letter must be completed by one College of Education Instructional or Administrative staff member, postmarked no later than February 1, 2008.
5. Applicants must be available for an interview either in person or by phone for this scholarship.
6. The application must be returned postmarked no later than February 1, 2008, to:

7. Recipient is requested to attend a luncheon in April 2008, to be recognized for the award.
8. Payment will be made after Fall 2008 enrollment verification is obtained.
